Today, more and more people prefer virtual therapy services over in-person therapy. “The use of online therapy over the COVID pandemic seemed to normalize its use moving forward, and increased accessibility has provided people who would not have sought out therapy a safe way to start,” says Rychel Johnson, LCPC. “Younger generations often utilize technology and seem more comfortable with online therapy services.”
In addition, research shows that telehealth can be just as effective as face-to-face services. (1) Our testing team has evaluated 12 well-known remote therapy services and ranked them on a scale from zero to five stars. Each score was specifically calculated based on provider availability, cost, user popularity, and types of service provided.

Therapist quality—30%
A top-notch teletherapy service should feel both personal and professional, not impersonal, or generic. It should connect users with certified and experienced therapists, not unqualified individuals. Here’s what we look for:
- Expertise
- Personalization
- Interaction quality
Accessibility and convenience—30%
Teletherapy directly impacts an individual’s ability to seek, engage with, and benefit from mental health services. Whether it’s a scheduling issue, lack of transportation, or being physically unable to see a therapist in person, everyone’s situation is different. With the convenience of teletherapy being a significant advantage, we evaluate:
- Ease of use
- Availability
- Platform reliability
Cost and value—20%
Unfortunately, mental health services are not a financial privilege for most people who don’t have insurance that covers it. In an era where mental health care is more necessary, yet often more expensive than ever, assessing the cost-effectiveness of teletherapy services becomes not just relevant but essential.
Here’s what we factor into a teletherapy’s cost and value rating:
- Pricing transparency
- Insurance and coverage
- Value for money
Support and resources—10%
Having additional support and resources is invaluable for clients seeking to deepen their understanding of mental health issues, learn additional coping strategies, or continue therapeutic work in between sessions. Beyond the therapy sessions, additional support can enhance the therapeutic experience.
Our factors in this rating include:
- Supplementary materials
- Customer support
- Aftercare
Privacy and security—10%
While teletherapy is an incredibly convenient way to seek mental health, it poses some risks. Maintaining client confidentiality and data security is paramount in teletherapy. As clients entrust teletherapy services with their mental health information, stories, and vulnerabilities, the responsibility of these platforms to protect such information is paramount.
Here’s what we’re looking for:
- Compliance with HIPAA and data protection laws
- Encryption and security measures
- Anonymity options

Best online therapy service for availability: BetterHelp
Key product features
What you should know
BetterHelp is one of the most well-known online therapy service providers worldwide. It matches clients quickly based on their preferences and availability and offers flat-rate pricing each month.
- Excellent for clients needing services in either the early morning or late at night
- Ability to receive services via four different options (messaging, live chats, phone calls, and video)
- Cash-pay only (can’t use insurance)
- Does not prescribe medication
Why we like BetterHelp for availability
BetterHelp claims to be the world’s largest online therapy service provider and advertises having over 30,000 mental health professionals on its platform. BetterHelp claims that, depending on the availability of providers on its platform, users can match with a therapist in a few hours or days.
We found that BetterHelp is intuitive to use, and it was straightforward to schedule an appointment. Therapists are independent contractors, so they set their own schedules, but users can often find availability from early mornings until 10:00 pm, which provides more access to those who are busy during standard working hours.
BetterHelp vets all therapists on the platform and requires them to be licensed or accredited, depending on their clinical titles. You can receive therapy in four different ways: message exchange, live chat, phone calls, and video conferencing. If you don’t like your current match, you can switch providers at any time.
A BetterHelp subscription consists of one weekly online therapy session in your preferred format, along with unlimited messaging between sessions. BetterHelp costs $70-$100 per week, but there is an option to apply for financial aid (although it’s unclear who qualifies). People are generally matched within a few hours or days after signing up, and you can read your therapist’s reviews after being matched. If, for any reason, you want to switch providers, customers have cited that the process for doing so is easy.
One important factor to consider is the Federal Trade Commission’s concerns about how BetterHelp has managed private health information. (2) In 2023, it ordered BetterHelp to pay a $7.8 million fine to customers for sharing health data for advertising purposes, including sharing email and IP addresses with Facebook, Snapchat, and other social media networks. (3) BetterHelp claims the practices that ended in 2020 are now cooperating with the FTC and have been certified by the information protection compliance service HITRUST. (4)
To learn more about how BetterHelp compares to other platforms, see our BetterHelp review.
What customers are saying
BetterHelp averages 4.5 out of five stars on Trustpilot, with 84% of respondents giving the company five stars.
However, Trustpilot also indicates that BetterHelp may be using ‘unsupported invitation methods’ to solicit this positive feedback.
“Two weeks of nothing: Takes two days to find a therapist, then when they assign you someone, they have such limited spaces available. But when you find availability you can work with, the next day it’s canceled with no follow-up message. By the time this has happened, you’re two weeks into your membership without having used a single function of the app or a single message from a therapist.”
-Sophie B., Trustpilot
Reviews on Reddit are mixed, with some users liking BetterHelp for its convenience and accessibility and others reporting high therapist turnover and mediocre mental health support. Many mental health professionals have expressed concerns about how BetterHelp manages user data and have vocalized dissatisfaction about the low compensation.

Best online service for therapy and medication: Brightside Health
Key product features
What you should know
Brightside Health is an integrative service that offers four types of mental health treatment plans. They accept insurance and are eligible for adults experiencing mild to moderate mental health issues.
- Acts as a one-stop-shop for clients needing both therapy and psychiatric medication
- Four plans with varying costs to fit different needs
- Best for clients intending to use health insurance, including Medicare & Medicaid
- Not recommended or appropriate for clients experiencing severe mental health issues.
Why we like Brightside Health for therapy and medication
Brightside Health provides individualized therapeutic and psychiatric support. This can be especially important for people with moderate or severe mood or anxiety disorders.
Brightside Health is an online therapy service that takes insurance, including Aetna, Cigna, BlueCross BlueShield, and some original Medicare Part B plans. You will need to fill out a form on its site to determine your insurance eligibility.
Therapy sessions are a standard 45 minutes, and you can choose your own therapist. All new patients are required to fill out mental health assessments, which can rule out anxiety and mood disorders.
Therapy plans include four video sessions at $299 per month. Psychiatry plans are $95 per month and include a psychiatric evaluation and messaging with your online psychiatrist. You can also opt for a therapy and psychiatry plan at $349 per month. Patients can purchase supplemental therapy sessions with their provider at $59 per session.

What customers are saying
Reviewers on Trustpilot value Brightside for being compatible with their insurance and its ability to prescribe. Reviews also mention being able to see a therapist much faster through Brightside than they could locally.
However, Trustpilot gives Brightside Health just a 1.5-star review out of five, with 87% of respondents giving the company just one star. Reviewers criticized the company for overbilling, difficulty with cancellations, and problems getting in touch with customer service.
Verified Trustpilot reviewer Kelsey Farwell stated, “If you want a robot to teach you about your emotions, choose Brightside. Therapy is by AI, peppered with an hour of chit-chat here and there that doesn’t remotely touch on everyday things that trigger you. It is not effective. You will be inundated with “therapy” videos narrated by a robotic voice, as if it couldn’t get any less intimate.”

Best online psychiatry service: Talkiatry
Key product features
What you should know
Talkiatry is an online psychiatric platform that works with people of all ages. It specializes in a variety of mental health conditions, including mood and anxiety disorders.
- Currently offers virtual psychiatry in 43 states
- Must use health insurance to participate in this service, pay for all appointments as needed
- One of the only platforms that works with young children (over age 5)
Why we like Talkiatry for online psychiatry
Talkiatry is in-network with most insurance companies, including Cigna, Aetna, BlueCross BlueShield, United Healthcare, Humana, and Medicare Part B. The cost of each service depends on your current insurance provider and individual plan details.
Talkiatry is also one of the only online psychiatric platforms that works with young children. It specializes in providing treatment for most mental health conditions, including depression, anxiety, ADHD, PTSD, and more.
Talkiatry is intended for patients experiencing mild or moderate mental health issues. It is not an online subscription therapy service. Instead, you pay for each appointment. It does not currently offer a self-pay option, so you can only use Talkiatry if it accepts your health insurance plan.
For more resources, check out our full Talkiatry review or our best online psychiatrists list.
What customers are saying
Trustpilot rates Talkiatry 1.5 stars, with 89% of the reviews being just one star.
Some of the complaints discuss poor communication, overpriced services, and issues navigating insurance and managing billing.
“After completing a survey and speaking to someone on the phone in order to select the right therapist, we were told by said therapist that Talkiatry can not accept her. They were in our insurance network and I got confirmation from Talkiatry in writing that we would only owe a $20 copay. We were then billed $427 for 2 sessions and a patient portal, none of which we received. It has been a year of contacting both customer service and billing to be told repeatedly they would take care of it just to get sent another bill the next month. Lack of transparency, care, and follow through coupled with fraudulent billing.”

Best online therapy service for messaging: Calmerry
Key product features
What you should know
Calmerry prides itself on its unlimited monthly messaging service. Clients can reach out to their therapists whenever they want, and they can expect to receive 1-2 responses per day during their therapist’s typical working hours.
- May be ideal for clients who prefer writing over talking
- Affordable monthly packages are available
- Therapy sessions are 30 minutes instead of standard 45-55-minute sessions.
Why we like Calmerry for messaging
Calmerry is a fairly new online therapy service that was founded in 2020. This platform offers a plan that allows for unlimited monthly messaging. Although this type of therapy may be more unconventional, emerging research supports the efficacy of messaging-based treatment. (7) With Calmerry, you can contact your therapist at any point between sessions. You can expect one or two responses per day during your therapist’s working hours. You also have the option of scheduling a 30- or 60-minute video session.
Calmerry members are encouraged to use the messaging app to write down thoughts, track progress, or seek support. The program also has an in-app mood tracker and journaling prompts that you can keep private or share with your therapist.
Calmerry offers an unlimited messaging plan that costs $200 per month. You can also schedule one live video session per month for $268 per month or four live video sessions for $295 per month. Calmerry offers financial aid, but it’s not clear who applies. Finding exact pricing is difficult on this site.
What customers are saying
Because Calmerry is newer, there isn’t significant customer feedback. The positive reviews highlight the therapist’s quality and the unlimited messaging feature.
Negative reviews state that the messaging service wasn’t robust, and some people indicated issues with matching therapists or only receiving canned responses from their providers.
“I signed up after the death of my mom after experiencing horrible grief and feelings of guilt. I would send large paragraphs of info about my life and feelings and would get a few sentences in return. After the month of ‘therapy’ was nearly completed I got the wonderful counseling advice of ‘Have you ever tried to think good thoughts?’”

Sesame Care has a 4.1-star rating on Trustpilot, with 77% of respondents giving the company five stars. Reviewers like Sesame for its intuitive platform and reasonable costs. Many referenced good-quality providers who were responsive and attentive.
“I tested positive for COVID this morning and could not get a virtual appointment with my healthcare provider today. It was straightforward to book an appointment with Sesame and Dr. Raphael Odekunle was pleasant and helpful. He sent a Paxlovid prescription right away after asking health and medical history questions. He also gave some medical advice.”
-Lori, Trustpilot
Some of the negative reviews state that they felt rushed by the doctors and experienced difficulties with billing.
“Sesame care is careless, vague, and unprofessional. I contacted a doctor here for telehealth. I was shocked after my appointment ended after just 10 minutes of sitting in front of a screen, with the doctor being very vague and quiet. He didn’t provide me with any help, information, references, or even take any notes. Best online therapy service for teens: Teen Counseling
Key product features
What you should know
Teen Counseling is specifically intended to treat teenagers and their families. Family members can attend their own sessions or participate in family therapy with their teenagers.
- Excellent for teenagers and their families needing mental health support
- Ability to receive services via four different options (messaging, live chats, phone calls, and video)
- Cash-pay only (can’t use insurance)
- Does not prescribe medication
Why we like Teen Counseling for teens
Many online services are only available to adults ages 18 and up, but Teen Counseling focuses exclusively on the well-being of teenagers and their families, though it does require that a parent or guardian enrolls in the sign-up process. While a teenager can initiate registration, an adult is required to confirm their presence before initiating treatment.
It’s a convenient and accessible option that emphasizes treating common teen issues, including anxiety (8), depression, self-identity, bullying (9), eating disorders (10), anger management, and trauma.
Live therapy sessions are only 30 minutes, which can be advantageous for teenagers who may not want to sit for an entire hour-long session (although this can be a downside to some people). Members can also send private messages to their therapists and schedule live chats. Like its parent company, BetterHelp, Teen Counseling ranges from $65-90 per week billed once per month. Financial aid is available, but there are no specific criteria regarding eligibility.

Best nonprofit online therapy service: Open Path Collective
Key product features
What you should know
Open Path Collective is a well-known platform where therapists agree to provide discounted therapy services to those in need of financial support. This is a nonprofit aimed at improving mental health accessibility. Many therapists also prefer using this platform over other online models, as there is no ongoing subscription, and they keep their full session fee from clients.
- Best for clients who can’t afford private therapy and have an annual household income below $100,000
- Can’t use insurance- rates range between $40-$80 and are negotiated directly with your therapist
- Choose your provider with both telehealth and in-person options available
Why we like Open Path Collective as the best nonprofit
Open Path Collective mimics a standard therapist directory. Unlike other online therapy options, Open Path Collective simply represents a hub of therapists agreeing to work at a reduced rate for clients seeking affordable therapy. All interactions happen directly with your therapist, and you will collaborate to establish session frequency and cost. You can also choose to pursue either virtual or in-person therapy sessions.
In addition, Open Path Collective is a nonprofit, which means membership costs go toward maintaining this organization. Unlike other therapy platforms, because these providers are not contracted, your therapist retains their full session fee from you. Individual sessions range between $40-$70, and couples/family sessions can cost up to $80 per session. You and your therapist will negotiate this rate together.
What customers are saying
Reddit reviews are fairly positive, with many appreciating the low-cost, transparent pricing and the ability to schedule in-person sessions.
While there are no TrustPilot reviews to date, Reddit user “hren018” reported, “I’ve had an amazing experience with my therapist through Open Path. I pay $50 a session for a fully licensed counselor with over five years of experience. I prefer it to BetterHelp which was $65 a session for me in 2019. I had a good therapist on there but she left the platform very unexpectedly. With Open Path, I narrowed down my search between three in my area that specialized with my issues, picked one, and have never looked back!”

What is online therapy and how does it work?
Online therapy is similar to in-person counseling in the services that are provided. The main difference is that online therapy is conducted virtually through video conferences, phone calls, mobile apps, texts and emails.
All online therapy services match clients with providers based on location, specialization, and specific preferences. You will generally meet with your assigned therapist once per week, although this frequency may vary based on your goals and treatment plan.
How to choose the best online therapy platform for you
Choosing an online therapy platform is an extremely personal decision. Although it can feel overwhelming, especially if you’re new to therapy, there are some recommendations and guidelines that you can follow that can help you find the right service.
Type of therapy
You’ll want to narrow down the specific type of therapy you need. Most online platforms offer encompassing one-on-one sessions, but if you’re looking for help in areas such as family therapy, couples counseling, group therapy, or trauma-based counseling, you’ll want to make sure it is available from the service you have chosen. Also, if you think that you may need medication as part of your treatment, look for platforms that have psychiatrists or medical doctors on staff.
Scheduling
Online therapy can be more convenient for many people because it is done virtually. However, when you are choosing your therapist, make sure that your schedules are cohesive or that they offer the flexibility you require.
Session format
You’ll get the most out of your therapy sessions if you’re comfortable with the communication style and feel like you can create a strong connection with your therapist. Many platforms offer various ways to connect with your therapist, such as video conferencing, phone calls, live chat, or asynchronous messaging. Be sure that the online service you choose offers your preferred session format.
Cost and insurance coverage
The cost is a critical element when reviewing your online therapy options as you want to make sure the service you choose is one that fits your budget so you can continue care as long as you need it. Many platforms offer upfront pricing for out-of-pocket costs for single sessions or subscription plans. We also encourage you to contact your insurance company to discuss whether online mental health treatment is covered and if your plan has any preferred providers or platforms.
Pros and cons of online therapy
How much does online therapy cost?
The costs of online therapy vary, and finding the exact amount you’ll pay can be complicated. Most general online therapy services cost between $40-$100 per session. However, this can change depending on your insurance plan and what kind of therapy you are undergoing, as specialists and psychiatrists may cost more than group therapy or peer counseling. If you’re choosing to see a therapist in private practice, fees generally range between $100-$250 when paying out-of-pocket.
Does online therapy take insurance?
The short answer is yes, many online therapy providers take insurance. Unfortunately, it’s not that simple. Insurance providers are increasingly covering online therapy, but the platforms and services covered can be vastly different depending on your specific insurance plan.
Also, online therapy regulations vary from state to state; some states require that online therapy and in-person therapy have the same coverage, but others do not have this mandate. The level of coverage and types of treatments covered may also change depending on your provider and plan. When it comes to out-of-pocket costs, co-pays, and deductibles and if your preferred online therapy platform can affect what you pay.
Does Medicare or Medicaid cover online therapy?
As telehealth continues to rise in popularity, Medicare Part B is covering more of these services, including online therapy. To use your benefits, you’ll need to first ensure the platform and provider you choose accepts Medicare—notable options include Brightside and Talkiatry. When it comes to cost, you’ll first be responsible for meeting your Part B deductible, then you’ll pay 20% of the Medicare-approved amount for your mental health provider’s services. (12)
Medicaid beneficiaries will be happy to hear that, as the single largest payer for mental services in the U.S., Medicaid includes online therapy as a covered service. (13) However, because these benefits are administered on a state-by-state basis, eligibility for teletherapy will depend on where you live. We recommend visiting your state’s Medicaid website to verify your coverage and limitations. If you’re covered, you can expect to pay the same for remote therapy as if you saw your therapist in person.
What to consider before purchasing an online therapy subscription
Before starting therapy online, consider your goals and if you have any specific preferences for your provider. For example, you may wish to work with someone close to you in age or someone who shares your religious background. You’ll also want to consider which communication options you’ll have available to you and how often you’ll meet with your therapist. Finally, look into cancellation policies. Keep in mind many online therapy services offer trial periods where you can try their service before committing to it.
Online therapy vs in-person therapy
Online therapy has been shown to work as well as in-person sessions, but there are still some differences to consider. In-person counseling sessions require participants to meet their therapist at an office, whereas online counseling is conducted through various virtual platforms, including video meetings, telephone calls, mobile apps, texts, and emails.
There are benefits and drawbacks to online and in-person therapy. When you meet with your therapist in person, they can more effectively read your body language. Also, there are fewer privacy concerns and insurance red tape with in-person therapy. However, online therapy is convenient, as most people can easily do it from home.
Are there privacy concerns with online therapy?
One downside to online therapy is potential security threats that could compromise your privacy. Online therapy platforms typically go to great lengths to keep your data safe from viruses, hackers, and thieves. Patients can also take steps to help protect their privacy.
Some of these guidelines include:
- Checking that your platform complies with the federal Health Insurance Portability and Accounting Act (HIPAA)
- Update your software and applications frequently
- Use data encryption and strong passwords
- Be mindful of what you share on social media

How to prepare for your first online therapy visit?
It’s understandable if you’re nervous about your first online therapy visit, especially if it’s your first foray into counseling. However, there are some steps you can take to help put you at ease. Before you start, jot down your symptoms, questions, and concerns so you don’t worry about forgetting anything. You can also be mindful of your physical space. Having a quiet, relaxing room can get rid of external distractions. If you’re nervous about the technology or your equipment not working, test it out beforehand.
Who should not use online therapy?
As you consider whether online therapy is right for you, assessing whether your circumstances and personal preferences align with this form of counseling is vital. For some individuals and situations, including those outlined below, participating in therapy online is not the most appropriate choice.
- Thoughts of suicide or self-harm: Most online therapy platforms cannot provide the level of support individuals in this state need. If you or a loved one are in crisis, it’s essential to seek immediate help by calling the crisis line at 988, texting “HOME” to 741-741, or calling 911.
- Severe mental health conditions: Online therapy is not recommended for individuals experiencing more serious mental health concerns, who typically benefit from a more intensive and directly supervised treatment plan offered through in-person therapy options.
- Unreliable internet connection or technology challenges: Frozen or lagging video, broken up audio, or difficulties connecting can cause significant disruptions and distractions that can impact the overall experience and effectiveness of online therapy.
- Lack of access to a confidential space: Privacy is critical when participating in mental health services. If you cannot find a confidential space when joining your remote therapy sessions, you may not feel comfortable or safe opening up and fully engaging in the therapeutic process.
- Difficulty connecting with others virtually: The relationship between you and your therapist plays a considerable role in whether therapy is successful or not. If you find it challenging to feel connected and build rapport with others online, teletherapy might not be your best option.

What is the best online therapy service that can prescribe medication?
We like Brightside, Talkiatry, and MDLive for their medication options. Brightside offers various subscription plans for customized care and works with many major insurance companies to help reduce the costs of therapy and psychiatry. Specializing in psychiatric services, Talkiatry is another excellent option for individuals of any age as one of the only platforms that offers medication management for children as young as five. MDLive is another favorite because it provides well-rounded wellness care with therapy, psychiatry, and urgent care services.
Are online therapists legit?
Online therapists must adhere to their state’s licensing requirements, which include accruing a designated amount of clinical hours, successfully passing all board exams, and keeping their license in good standing. You can always look up a provider’s license by looking online at their licensing board’s website.
“A quality therapist will be transparent about their experience, credentials, and approaches they utilize when working with clients,” Johnson says. “Trust your gut, if something feels ‘off’ about a therapist, you have a right to seek out a different provider.”
Is online therapy confidential?
All therapists are legally required to maintain your confidentiality. This applies whether they meet with you in person or online. However, there are some exceptions to confidentiality, which include reporting instances of child abuse, dependent adult abuse, and elder abuse. If you have a plan and intent to hurt yourself or someone else, your therapist will also need to breach confidentiality. If you’d like your therapist to consult with other healthcare professionals on your behalf, you can have them sign a written release of information.
Is online therapy worth it?
Online therapy can be just as effective as in-person therapy, and with the added benefits of convenience and affordability, we feel it absolutely proves its value and worth. (11) Choosing a reliable platform with vetted and qualified therapists can make a significant difference in your experience. However, online counseling is not the best solution for everyone, so you should consider your mental health needs and preferences to determine if it’s suitable for you.
What is the largest online therapy platform?
BetterHelp’s global reach, with over 30,000 mental health providers and availability in 210 countries, makes it the largest online therapy platform we’ve evaluated. Its wide range of services, including individual, couples, and teen counseling, can accommodate many users’ needs. Other top contenders include Talkspace and Brightside, which both offer extensive therapy and psychiatry services through large provider networks.
Is online therapy expensive?
The cost of online therapy can vary greatly depending on the platform, the services you choose, the quality of the therapist, and your insurance coverage. You can find reasonable session costs ranging from $40 to $100 through subscription models, insurance coverage, or sliding scale fees. For comparison, in-person therapy typically costs between $100 and $250 per session.
